Access Medical Records
The practice is registered and complies with the Data Protection Act 1998. Any request for access to medical records by a patient, patient’s representative or outside body will be dealt with in accordance with the Act.
A lot of your information is available to you on the NHS app. If you cannot access this, and wish to request a copy of your medical records held by the practice, click here to download and complete a Subject Access Request/Access to Health Records Act Application Form. If you cannot print the form, you can collect a copy from reception.
Please return the completed form to reception. You must provide 2 forms of I.D (1 photographic I.D, such as drivers licence or passport, and 1 proof of address, such as utility bill or bank statement).
Please be aware there may be a charge. All relevant sections of the form must be completed including the reason for application. Forms that are incomplete will not be processed.

Breast Screening (Mammogram)
When you’ll be invited for breast screening and who should go
Anyone registered with a GP as female will be invited for NHS breast screening every 3 years between the ages of 50 and 71. You’ll get a letter in the post inviting you.

Childhood Immunisation Clinics
Pear Tree Surgery do the following childhood immunisations: 8, 12, 16, 20 week, 12 month and 3 year 4 month (preschool booster) immunisations.
You must have a doctors appointment prior to having 8 week immunisations.
Parents please note if sending children with grandparents or any other member of your family you must provide consent for the nurse to administer immunisations.

Counselling
The practice has CBT workers/counsellor visiting from the talking therapies service: Self Referral | Coventry, Warks & Solihull Talking Therapies.

Diabetes Clinics
The diabetic clinic is supervised by a senior GP partner and run by our nursing team. The nursing team have attended specialist diabetes training and accreditation in order to deliver this service.
Patients at Pear Tree Surgery who are diagnosed with diabetes will be invited for an annual review. The administration team at Pear Tree Surgery will invite each diabetic patient annually to have a blood test and book an appointment at the surgery. If you are overdue a review and have not been invited please contact the surgery to allow us to investigate.
It is important you attend annual review to ensure your lifestyle and medications are working optimally for you.
Please specify to reception that you are attending for a Diabetes review, as not all nurses are diabetic trained.

The practice has its own dispensary and the staff take part in the Dispensing Quality Service scheme.
The areas that we can dispense to are: patients who do not live within one mile of a pharmacy. Some of these areas are: Hurley, Wood End, Piccadilly, Lea Marston, Marston, Bodymoor Heath, Whitacre Heath, Nether Whitacre, Baxterley and Curdworth
If you do live within one mile of a pharmacy, we are unable to dispense to you.
We can provide the medicines you need either direct from the surgery or delivered to your home.
Please provide evidence if you are exempt from charges the staff will need to see this evidence each time you collect your prescription. Electrocardiogram (ECG)
An ECG (electrocardiogram) is a test that records the electrical activity of your heart, including the rate and rhythm. It’s usually quick and painless.
Following an appointment with a doctor you will be advised to book in for either an ECG or 72-hour ECG.

Ear irrigation
It’s important that the wax is soft to make the procedure as comfortable and effective as possible. We recommend using medicated olive oil drops, once per day in the affected ear and repeat for 2 weeks before your appointment.

Family Planning/Cervical Smear Tests
Contraception
The practice offers a full range of family planning services including prescriptions for oral contraceptives, morning after pill, implants and coils.
Starting contraception: If you require an appointment to start contraception, please book this with a Doctor.
Contraceptive pill or patch: If you currently use the contraceptive piil or patch and wish to continue, this can be booked with the practice nurse.
Depo injection: If you are currently having the contraceptive depo injection, please book an appointment with the practice nurse.
Coil or Implant: If you have had an appointment with a doctor and have discussed having a coil or implant, please book an appointment in our gynae clinic with Dr Fiona Thomas or Dr Jennifer Stone

Smear tests involve an examination of the cervix (the neck of the womb) to check for warning signs which may occur before cancer develops. We recommend that all women between the ages of 25 and 65 should have regular smear tests.

Flu Clinics
These are offered each winter from early October. The vaccination is particularly recommended for patients with asthma, heart disease, diabetes or suffer with recurrent bronchitis, or those patient who are over 65 years of age.
We hold flu clinics on selected dates from September onwards as advertised by practice. Please do not wait for contact from the practice if you are over 65 or eligible for flu injection.
Children covered in age cohort for NHS nasal flu will be contacted by the surgery and an invitation will be sent out via SMS.

Health Visitor
If you are expecting a baby or have a child aged 0-5 the health visitor can offer you a range of advice and support from breast feeding and weaning to immunisation advice and minor illnesses.
You will also receive 5 key visits:
– 10-14 days after the birth of your baby
– When your baby is 6-8 weeks old
– A review of your child’s development at 9-12 months
– A review of child’s development at 2 years

The doctors undertake minor surgical procedures at Pear Tree Surgery.

The majority of procedures, consultations and services carried out at this surgery are paid for by the NHS. However, where additional services are provided, we are required to pass on the specific cost to patients.
The type of additional, non-NHS funded services most frequently requested include:
– insurance forms
– holiday cancellation forms, medical reports
– fitness to travel certificates
– private prescriptions
– hgv/employment medicals
– some vaccination services
Our fees for these services, which are in line with BMA approved national guidelines, are displayed in the surgery. Please inform the reception staff if booking for medicals as these are not booked in to routine appointment slots.

If your symptoms are deteriorating, or you are having any concerns, please make a face to face appointment with our nurse.
If you have received a letter inviting you to make an appointment for an asthma review, please do so. Depending on the type of medication you currently use, it is not always appropriate to submit the review form online. You will need to see or speak to a GP or nurse if you have been asked to.

Tetanus Vaccination
A booster may be needed for patients over 15 years old if they completed a tetanus course more than 10 years ago. If you completed a course and think you may need a booster please check with a practice nurse before booking an appointment.
Travel Health
If you are going on holiday and believe you require vaccinations, please book a telephone consultation with the practice nurse to discuss what vaccines you require. The practice nurse will then order in any vaccinations required and you will be booked in for a face to face appointment to receive those vaccinations.
Please note that each patient will need to book a separate appointment and some vaccinations may incur a charge as may not be not covered by NHS.
